Adult: $329.00/year
Child: $229.00/year (under age 14)

Coverage is in effect for one year from in house start date. Coverage begins when membership has been paid in full. Appointment times are limited, and it is the responsbility of the member to schedule all appropraite visits. The plan is not transferable and if dental insurance benefits are obtained during the term of the plan, the plan will become ineffective and no refunds will be given. If benefits under the Plan are not taken advantage of for whatever reason no refund will be given.
